In order to facilitate a more effective case handling process at the Faculty, streamlined forms are mandatory for applications in typical student affairs.
Use the forms available from WebDziekanat, which you can generate from there or use one of the following:
- Student ID Card duplicate issuance
- Request for information collection in a business for diploma thesis or project purposes
- Student final settlement card
- Request for study fees reduction for foreign students
- Student declaration to allow third parties to access information about their study progress
- Documents and diploma receipt power of attorney
- Resumption of studies
- Change of diploma thesis supervisor.
Dear Student, remember that:
- a form must be completed clearly and contain a reason
- a free form must be used if your case does not have a relevant form available
- an application form to the rector must be used only to complain about a Vice-dean for Student Affairs’ decision about removal from the list of students.
- an application for reduction of missing ECTS credits fees/educational services fees with the opinion of the Faculty’s student government must be submitted not later than 7 days before the beginning of the semester or the occurrence of the event constituting a basis for the reduction.
Submit the carefully prepared form with attachments in person during the Dean’s Office opening hours, deposit it in the dropbox in the Dean’s Office or send it via regular post to the Dean’s Office. If your case requires immediate action, report for the nearest consultation hours of the Vice-dean for Student Affairs.
